Adrenochrome was nothing but an obscure laboratory chemical that had at one time been thought to influence schizophrenia until Hunter Thompson popularized it as a powerful hallucinogen in a one-off reference in "Fear and Loathing in Las Vegas". He was probably under the influence of more conventional psychoactives at the time.

Yeah - it's a completely made up drug and yet it's the whole backbone of the conspiracy.

Adrenochrome IS a real thing. Your body puts adrenaline into your bloodstream and your nerves and tissues react in certain ways to the adrenaline by capturing and binding with components of it as it passes over them. Adrenochrome is one of the things that's left over at the end of that process - it's basically waste, the leftovers. Your body eventually gets rid of it like all waste products. But it doesn't make enough of the stuff to be meaningfully "harvestable", and even if it did, there isn't actually a real existing way to extract it from peoples' bloodstream. But I mean, obviously that's not gonna stop people who believe that a literal Star-Trek like medical bed secretly exists that cures cancer just by you laying in it, so.

Wikipedia says

Hunter S. Thompson mentioned adrenochrome in his 1971 book Fear and Loathing in Las Vegas.[16] This is the likely origin of current myths surrounding this compound, because a character states that "There's only one source for this stuff ... the adrenaline glands from a living human body. It's no good if you get it out of a corpse." The adrenochrome scene also appears in the novel's film adaptation.[15] In the DVD commentary, director Terry Gilliam admits that his and Thompson's portrayal is a fictional exaggeration. Gilliam insists that the drug is entirely fictional and seems unaware of the existence of a substance with the same name.
