I had taken a course on workplace safety years ago (was a store manager) and we were taught about safety around ladders, among other things, to make sure our employees and customers were safe, etc.
We had high ceilings, like twenty feet or so, and had to replace the floodlights and such. Anyway, we had very high ladders, of course.
Well, no one wanted go that high so I always ended up doing it. I don’t mind heights, but yeah, make sure your ladder is up to code, and on solid ground. No matter how much you’re careful, though, those things are evil.
Also, I’ve seen people do this when they don’t have a high enough ladder.
